Cancellation Policy
Booking Policy. 1. Booking Requirements: • All appointments must be booked at least 24 hours in advance to ensure availability. • Same-day appointments may be available on a case-by-case basis; please call the office to check for availability. • A valid credit or debit card is required at the time of booking to secure your appointment. 2. Cancellation & Rescheduling: • Cancellations or rescheduling requests must be made at least 24 hours before your scheduled appointment. • Cancellations made less than 24 hours in advance will result in a $25 cancellation fee charged to the card on file. • No-shows will be charged the full price of the scheduled service. 3. Late Arrival Policy: • Please arrive on time for your appointment. • If you arrive more than 10 minutes late, your appointment may need to be rescheduled, and a $25 late fee may apply. • If you are running late, please call the office to notify us as soon as possible. 4. Payment & Insurance: • Evolve Hydration and Wellness is a cash-based practice. • We do not accept insurance for any services. • Payment is due at the time of service. • We accept cash, major credit cards, and debit cards. 5. Refund Policy: • Services are non-refundable once provided. • If you experience an adverse reaction to any treatment, please contact us immediately for guidance. 6. Health Considerations: • Clients must complete a health intake form before receiving any treatment. • If you are pregnant, breastfeeding, or have any underlying medical conditions, please inform our staff before booking. • Evolve Hydration and Wellness reserves the right to decline service if it is determined to be unsafe based on the client’s medical history. 7. Age Requirements: • Clients must be 18 years or older to receive IV hydration or vitamin injections. • Clients under 18 require parental consent and must be accompanied by a parent or legal guardian. 8. Group Bookings: • Group appointments for events, parties, or wellness sessions must be booked at least one week in advance. • A 50% deposit is required for group bookings and is non-refundable if canceled less than 48 hours before the event. 9. Concierge Services: • Concierge services are available for an additional fee based on travel distance. • Please inquire about availability when booking. 10. Right to Refuse Service: • Evolve Hydration and Wellness reserves the right to refuse service to anyone at the discretion of the medical staff if it is deemed unsafe.
